Pope Leo XIV remains in charge during surgery in Rome

Pope Leo XIV continues to run the Catholic Church during surgery in Rome, retaining full governance authority under canon law while hospitalized.

Pope Leo XIV arrived at Gemelli hospital in Rome on Tuesday for surgery. The pontiff keeps full governance authority over the Catholic Church while unconscious under general anesthesia.

No legal provisions exist to transfer papal authority during illness. A lung nodule prompted the procedure. Leo expects a weeklong stay.

The pontiff is the 267th pope. He won election on May 8, 2025. Church canon law defines his position as successor of the Apostle Peter.

Vatican officials manage daily administrative functions. Vatican secretary of state Cardinal Pietro Parolin spoke at a nearby conference Tuesday.

Parolin described the operation as unexpected.

“We were all struck by the serenity with which the pope announced this procedure,” Parolin said. “Am I worried? Yes, because I certainly didn’t expect something like this.”

Parolin spoke with the pope Monday night. The meeting reassured him.

“It’s something that, thank God, we noticed right away. And normally, when these things are caught in time, they can be resolved,” he said. “But these are, as the pope said, God’s surprises.”

Two new bishop appointments moved forward Tuesday. Other events adjusted schedules.

Church governance and canon law

Canon 412 allows a diocese declaration as impeded during bishop incapacity. Auxiliary figures then assume local control.

Canon 335 addresses a vacant or impeded Holy See. It offers no explicit process for an incapacitated pope. What defines an impeded Holy See remains unstated.

In 1965, Pope Paul VI drafted letters about severe illness. The letters asked cardinals to accept his resignation if incapacitation struck. Paul died in office 13 years later.

Pope Francis confirmed in 2022 that he wrote a conditional resignation letter. He gave it to Cardinal Tarcisio Bertone. The letter went to the office of Parolin. The conditions were never made public.

Canon law demands that a papal resignation be freely and properly manifested. Pope Benedict XVI used that process in 2013.

Rules during papal transition periods

Power transfers only when a pope dies or resigns. That period is known as the sede vacante.

The chamberlain, known as the camerlengo, oversees Holy See finances during an interregnum. Cardinal Kevin Farrell holds that role. The camerlengo has no special authority during a pontiff’s illness.

Farrell is currently presiding over a Vatican conference on family ministry. Leo attended the gathering prior to his hospitalization.

An interregnum represents the formal gap between pontificates. The College of Cardinals convenes in a papal conclave to elect the next successor. Normal governance remains frozen until that conclave concludes.

Medical teams monitor recovery in Rome following surgical care. Official daily operations continue under existing staff.
